To ensure quality customer service, and that you receive all replacements at once, please fill out this form after you have taken inventory of your order and opened all boxes. Once the form is submitted with photos, and approved from our claims department, your order will be processed and submitted to the warehouse. **A picture of the damaged item/part is needed to process your replacement claim. To ensure a timely turn around please submit your photos at the same time with this claim form.
